Our verification process normally takes 24-48 hours for completion. Verifications requested after 2PM ET on Fridays or on holidays will be processed the following business day. Once you are an established client, submittal of the verification form is not required. Established clients may simply phone us.

If you can not or choose not to fill in a field marked by * please type NA or X in the field for your form to be submitted. Inaccurate submittals or incomplete data will only delay your verification.

"First Time" appointments require a 2 hour minimum. One hour appointments are only available for established clients in good standing.
